Each ministry is represented on our website's ministry landing page. In filling out this form, you are indicating you would like a separate webpage for your ministry. This page will be formed with our Mission and Evangelization coordinator's help and will require Fr. Jack's approval.

Please submit this form with at least 3 photos of your ministry at work. Photos must be clear, acceptable, and dynamic. Please be sure you have the approval of the individuals in the photos.

The information submitted here can help us construct your ministry's brochure as well.
